Does a Medical student need laptop or iPad for his study?

Definitely an iPad! 2-in-1 laptops aren't as good as it sounds. Their tablet features are not even close to an iPad. And considering you're an MBBS student, you'll spend more time reading books on it than making presentations. My friend owns a 2-in-1 laptop (HP Spectre x360) and it's quite heavy. It's not portable, trust me! And apple pencil is extremely helpful for you to take notes if you're interested in going paperless. But I strongly recommend you to buy an apple pencil itself and not any 3rd party styluses. Also you can always buy a mouse and a keyboard (not necessarily of apple's) for your iPad if you really want it. Although you need to take the drawbacks of owning an iPad into consideration - You cannot connect an external pendrive/harddrive to your iPad unless you're buying a pro model (pro models having type-c cable and not a lightning cable). But I don't think it'll be necessary for you if you're buying an iPad of 64GB or above. I would recommend you to not buy any iPad of 32GB. It's too less. There's no HDMI cable in an iPad obviously and hence if you have to give a presentation you'll have to borrow someone else's laptop. Don't worry, you can make your presentations on your iPad itself. All Microsoft apps are free in iPad OS. But honestly, if you can deal with the above problems then iPad is 100 times better than a 2-in-1 laptop. If you’re interested, check out these drawings done by my friends using the Apple Pencil on iPad Air 3. Precision at its best! You’ll be awestruck by the smoothness of the pencil once you use it.
